How we collect Personal Information
The primary source of personal information is direct interaction with individuals, whether electronically, in person, or over the phone. Additionally, information is gathered from referring clinicians, allied health professionals, family members (where appropriate), and other third parties, ensuring compliance with workers compensation claims, Medicare, and other relevant entities.
Types of Information Collected
We collect information including, but not limited to, names, contact details, dates of birth, addresses, medical history and identifiers such as Medicare numbers and expressed preferences regarding health services. Additionally, results of tests or procedures, payment details, and feedback are collected.
CRT will only collect health information that is necessary for the purpose for which it is collected and does so in a way that is lawful, fair and not unreasonably intrusive.
Use and Disclosure of Personal Information
Personal information is used for its primary purpose (medical care), service provision, communication, responding to queries or complaints, and facilitating billing. It may also be used for quality assurance, research, and statistical analysis where consent has been given. Disclosure may occur for administrative needs, accreditation, and legal requirements.

Online access to reports and images is facilitated through secure electronic systems with encryption. Access is granted to referring clinicians and other healthcare providers with explicit consent.
